Fit notes and proof of sickness

Employees must give their employer a doctor’s ‘fit note’ (sometimes called a ‘sick note’) if they’ve been ill for more than 28 days in a row and have taken sick leave. This includes non-working days, such as weekends and bank holidays.

If an employee started their sick leave before 10 December 2021, they will need a fit note if they’ve been off work for more than 7 days, including non-working days.

If employees are self-isolating and cannot work because of coronavirus (COVID-19) they can get an ‘isolation note’ online from NHS 111. They do not have to go to their GP or a hospital.

Fit notes are free if the employee has been ill for more than 28 days when they ask for one. The doctor might charge a fee if they ask for the fit note earlier.

If employees are off work for 28 days or less, they can self-certify. They do not need to give their employer a fit note or other proof of sickness from a medical professional.

Surgery opening on 24th and 31st December

Dear Patients 

From 13:00 on 24th December and 31st December we will be operating a normal service for the surgery from the Farnham Road Surgery site. The surgery telephone number remains open until 18:30 and other usual forms of communication, such as email, continue to operate. However, you will not be able to attend the surgery in person. We have taken this decision to reduce the risk of service interruption in light of the increasing prevalence of omicron and the risk of high rates of sickness within our team.

If you need to collect or drop off prescription requests or other paperwork please do this by 12:30 on the 24th and the 31st. In the afternoon you will be able to do this at Farnham Road Surgery.

If you already use patient access you can continue to use it. You can use the NHS App as well.
